PAA' Homeless Case Management and Outreach program provides comprehensive, community-based assistance to the homeless within the Polish community. PAA's homeless outreach workers initiate contact with Polish-speaking homeless in the streets. Winning their trust is crucial to helping them accept this much-needed intervention. Outreach workers discuss viable alternatives with the homeless persons, make arrangements for transportation to emergency housing, and help them access medical and social services. After an initial assessment of emergency needs, the caseworker either makes arrangements for the client to receive medical attention or directs the client to the Turning Point Day Shelter/Support Center, another collaborative shelter, or a transitional program, depending on individual needs. Case Management Staff works with collaborating providers at shelters, nursing homes and other agencies, and their Polish-speaking clients of both genders throughout the city of Chicago. Case managers arrange access to services and provide clients with interpretation during appointments with other service providers as necessary.
